Be the difference.Job SummaryProvides care and consultation for a specific patient population with alterations of wounds, the GI/GU systems, drains, pressure ulcers and incontinence. Serves as a resource for other health care professionals, patients, families and the community. Designs systems to facilitate the appropriate care of this patient population for the Greenville Health System.Accountabilities* Assesses the physical and educational needs of patients undergoing ostomy surgery or continent diversions, or -patients with wounds, fistulas, incontinence, percutaneous tubes or pressure ulcers. - Communicates patient needs to appropriate members of the health care team. - Provides discharge teaching as requested. - 20%* Identifies opportunities for system-wide improvement in skin, ostomy, wound or continence care; develops/revises Nursing Policies and Procedures, Standards of Care, and protocols based on current research. - Evaluates the outcomes and effectiveness of system wide improvements. 15%* Develops plans for care management based on patient needs through consultation and collaboration with other members of the patient's health care team; coordinates care in an efficient, cost effective manner. 15%* Based on protocols, procedures and guidelines, implements and evaluates the plan of care for individual patients.15%* Identifies actual/potential issues of resource management/patient care, works with health care team to identify cost-effective alternatives; facilitates discharge. 15%* Educates patients, families and staff on evidence-based skin and wound care practices. - Promotes health care, in area of expertise, in the community through education. - 5%* Promotes role identification by serving as clinical preceptor to undergraduate nursing students and staff. - 5%* Coordinates meeting agenda and issue follow up for the Prisma Health Skin Care Committee. - As a member of the Committee, evaluates products used, including specialty beds and mattresses and presents findings and makes -recommendations to Prisma Health Skin Care Committee and Products Committee. - - - -5%* Coordinates quarterly Pressure Ulcer Prevalence Studies and facilitates performance improvement activities related to analysis of results and serves as a resource. - 5%Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.Minimum EducationBachelor's Degree - NursingMinimum Experience3 years - RN experienceRequired Certifications/Registrations/LicensesRN - REGISTERED NURSE - NLRNCertified by WOCNCB as a CWOCN, CWON, CWCN or COCN (Lawson Code COTR)Current South Carolina R.N.
